    You need to fulfill the pre-requisites, that is, you need a PESEL and a zameldowanie. If you have both of these things, than theoretically you’ll need to send the filled out form to the address of the tax office (Urząd skarbowy) that corresponds with the address of your registration (zameldowanie). To see which Urzad Skarbowy you are under the jurisdiction of, here’s a list of them: http://www.pit.pl/urzedyskarbowe/

    You should call first and see if you can even send the form (some may require you to submit the document in person).
